The process of designing long-term care insurance (LTCI) systems is inherently
political, because governments must decide about trade-offs among groups with
competing claims on scarce resources. The choices are particularly difficult in
Germany and Japan's public LTCI systems due to low forecasts for economic
growth, shrinking workforces, rapidly aging populations, and limited fiscal resources.
This talk provides a comparative overview of the Japanese and German
systems and current reform proposals, emphasizing political constraints and opportunities.
